Fotona 4D combines four Er:YAG and Nd:YAG laser modes to address several features of facial ageing in a coordinated session. At Medical Aesthetics 360, the protocol is adjusted to the concern: mild laxity, perioral lines, texture, pores or general skin quality.
Why the layered approach matters
Fine surface roughness, enlarged-looking pores, softening around the mouth and early laxity are different problems. Fotona 4D uses four treatment directions so a session can address more than one of them without assuming that every face needs the same intensity.
How Fotona 4D works
The "4D" name describes four complementary laser modes. It does not mean four-dimensional imaging or a surgical lift.

The treatment can be compelling for a patient who wants several modest improvements in one plan. It should not be presented as a replacement for every structural or surgical option.
Who may consider treatment
Skin tone, tanning, cold-sore history and tolerance for peeling all influence which stages and settings are appropriate.

What current evidence can tell us
A prospective Fotona 4D study indexed by PubMed followed 13 Chinese women after four full-face sessions at one-month intervals. Three-dimensional imaging found small vertical changes at several facial landmarks, approximately 0.5 to 0.63 mm in the lateral periorbital measurements at selected follow-ups.
Nasolabial-fold ratings and pore scores also improved at three and six months. The study reported transient redness and swelling, but it had no untreated control group and a very small sample. It supports the possibility of early and mid-term improvement; it does not establish equivalence to a surgical facelift or predict how every patient will respond.
Practical interpretation: Fotona 4D is best described as layered laser rejuvenation with potential for subtle tightening, smoother texture and softer folds. The evidence does not justify promises of dramatic lifting or restored facial volume.
Choosing the right level of treatment
The useful choice depends on whether the priority is skin surface, mild multi-layer rejuvenation or structural repositioning.
What to expect at an appointment
The full protocol commonly takes around an hour or longer. Comfort and skin response are checked throughout rather than treating every stage at a fixed intensity.
Facial ageing pattern, skin tone, pigment history, oral health, cold sores and treatment goals are reviewed.
The skin is cleansed, eye protection is used and the sequence is adjusted for the planned coverage.
Intraoral and external stages are delivered with temperature, comfort and tissue response monitored.
Instructions reflect the final peel intensity and may cover cleansing, moisturising, heat avoidance and sun protection.
Treatment course and results
Some patients start with one Fotona 4D session and review the result. For broader ageing concerns, three to four treatments around four to six weeks apart are commonly considered. The recent prospective study used four monthly sessions.
An early glow or firmer feel may reflect surface change and temporary swelling. Collagen-related effects develop more slowly, with a better assessment generally made two to three months after completing the planned course. Maintenance can then be discussed according to benefit rather than scheduled automatically.
Recovery and safety
The external peel can be adjusted from very light to more active. That choice affects both surface improvement and how visible the recovery may be.

Fotona 4D is a four-stage facial laser protocol that combines 2940 nm Er:YAG and 1064 nm Nd:YAG wavelengths. It includes intraoral heating, targeted external treatment, broader dermal heating and a light surface peel to address different layers in one plan.
The name refers to four treatment modes rather than four physical dimensions. SmoothLiftin, FRAC3, PIANO and SupErficial are applied in sequence, with each stage serving a different intraoral, dermal or surface role.
The protocol combines Er:YAG at 2940 nm with Nd:YAG at 1064 nm. Er:YAG is used for the intraoral thermal stage and surface peel, while Nd:YAG supports targeted and broader external heating stages.
It may suit mild to moderate facial laxity, fine lines, selected nasolabial or perioral creasing, enlarged-looking pores and uneven texture. Pigmentation must be assessed separately because not every brown patch is appropriate for a general rejuvenation protocol.
It is frequently marketed that way, but it does not reposition deeper facial tissues or remove excess skin like surgery. Fotona 4D is better understood as a layered laser rejuvenation treatment that may produce mild tightening and textural improvement.
No laser can restore bone, fat compartments or precisely placed structural volume. Temporary swelling and softened folds can make the face look fuller, but marked volume loss usually requires a different treatment strategy.
Settings and the surface-peel component need to be adjusted for skin tone, tanning, pigment history and sensitivity. Melanin-rich or pigment-prone skin may require a more conservative plan; customisable settings do not make the risk of pigment change zero.
Comfort varies across the four stages. Patients may feel intraoral warmth, hot pinpricks or sustained external heating. Many treatments are performed without injectable anaesthetic, but the experience should not be described as completely painless for everyone.
A full appointment often takes around 60 to 90 minutes after preparation. Timing varies with facial coverage, settings, cooling, the intensity of the surface step and whether every stage is included.

Laser treatment around the mouth may reactivate herpes simplex in susceptible people. A history of cold sores should be disclosed before treatment so timing and any need for medical advice or preventive medication can be considered.
Possible risks include prolonged redness or swelling, cold-sore reactivation, burns, blistering, infection, acne flare, pigment change, scarring, uneven response or little visible improvement. Risk varies with settings, skin condition and aftercare.
